II: SIGN UP FOR FIELD ALERTS VIA TEXT by going to the FIELD STATUS BOX in the upper right corner of the WSA HOMEPAGE, or follow this direct link: WSA RAINED OUT TEXT ALERTS. This alert system is strictly tied to "Field Status".  When the "Field Status" of a WSA Facility is changed you will receive a text alert.

VOLUNTEER DISCLOSURE FORM
Must be completed by EVERY coach, manager, and any adult associated w/ team duties. Turn in to Mr. Lovelee.

USYSA PLAYER REGISTRATION FORM 
This form is used to register w/ WSA u11-u19 teams and requires a parent signature

COMPETITIVE TRYOUT COMMITMENT FORM
This form is required to register to a team.  Parent signature required.  Acknowledges 1-year commitment.

USYSA MEDICAL RELEASE FORM
This form is used for u11-u19 WSA teams.  One per player should be taken to each game & practice.

CLUB WIDE PLAYER PASS GUEST PLAYER FORM
This form is used for players utilizing the club wide player pass (approval through Mr Lovelee or Roger required)

SIT OUT VERIFICATION FORM FOR RED CARD/EJECTION
This form must be completed for player or coach serving a red card ejection.  

NOTICE OF LATE PAYMENT LETTER 
Letter from DOC reminding your parents that their fees are late.  Please send on the 6th of each month.

MANAGERS WEEKLY EMAIL TEMPLATE
Remind parents of upcoming payments, schedule, etc.  Fully customizable. 

GUEST PLAYER TRAVEL FORMS
Use this form to get permission for players to "guest play" with your team.  NOTE: WSA teams must contact the guest players coach directly to receive permision for use of a "guest" player.

DONATE TO WEST SIDE ALLIANCE S.C.

Managers please make your families aware of the option to donate to the West Side Alliance S.C.  Donations can go toward the general fund or donors can choose specific sponsorship options (i.e. uniform, registration, etc).  You can also use this donate button as the "buy-out" for the club fundraiser.
