TRYOUT PRE REGISTRATION FORM (OPTIONAL)

What is this? This form is designed to create an easier process for you and your family at the 2010 WSA Tryouts.  Complete the information on the form to indicate you plan to attend a WSA tryout.  If you are selected to a team will process your credit card information in our automatic billing system (you will be billed for the first time on August 5th, and on the 5th of the subsequent months of thru May).  You can suspend this service at any time.  Your information will not be processed if you are not selected to a WSA team or if you elect not to play for WSA.  

Do I have to complete this form? No. This is purely optional.
