STEP 2) Download Medical Release Form, Complete w/ Parent Signature (see Age Division Chart Below). If you have played in the Youth Development Program or WSA Academy Program prior to 2012 and have already submitted a Medical Release Form (aka, "US Club Registration Form") then you do not need to submit this form again.
