THREE REQUIRED FORMS TO BE MAILED TO THE CLUB

USYSA Registration Form * 1 of 3 forms required w/ signature (player can sign if 18 years old)

GCSA Tryout Commitment Form * 2 of 3 forms required w/ signature (player can sign if 18 years old)

Medical Release Form * 3 of 3 forms required w/ signature (player can sign if 18 years old)

DRIVERS LICENSE COPY or BIRTH CERTIFICATE COPY

Must mail a copy of your birth certificate or your driver’s license

PICTURE FOR PLAYER PASS

Must mail a small postage-stamp sized photo (head shot)

PAYMENT

To complete the registration you must include a check for $50 (make check payable to "WSA Soccer"

$50 Covers ALL Costs (registration, insurance, entry fee, uniform, training)  * does not cover hotel/travel for regionals

SCAN & EMAIL OPTION

Instead of mailing paperwork you can scan and email all documents including picture (formats accepted PDF or JPG) to RogerWSA@aol.com.

ONLINE PAYMENT OPTION

You can choose to pay online with credit card but must email RogerWSA@aol.com upon making transaction (to insure money is allocated toward your State Cup account).  Click button below ($5 transaction fee is added).
